Several clinical syndromes can present with the symptoms or signs resembling an acute stroke. A 52-year-old woman uncontrolled diabetes presented of a hyperacute right middle cerebral artery (MCA) However, there was discrepancy between patient’s and multimodal neuroimaging findings. Further diagnostic workup confirmed bacteremia cerebritis caused by <i>Klebsiella pneumoniae</i> liver abscess. Thus, we made diagnosis <i>K. abscess syndrome (KLAS). Following appropriate antibiotic treatment, patient recovered completely without any neurological deficits. This is novel report on in KLAS presenting MCA Clinicians should consider as tentative upon observing patients
